It's a 2 channel amp with built in electronic crossover. You don't use a passive crossover.

Even a small TableTuba will crush that PV1D, any of the full size TableTubas even more so, while a THT is to the PV1D as an Abrams tank is to a Mini Cooper. For the smallest footprint place them upright. I only use one TableTuba 2x4 and it's plenty.

The driver Fs does not need to be 19, it needs to be between 17 and 30. Lower Qts is better than higher. The cab width is explained in the plans. Placing subs below a TV is a very bad idea. Classic rock cuts from before the 80s weren't mixed with much low bass as few listeners had gear that would handle it, especially as the #1 payback medium was car radios. Remixes done from the 90s on have much better low end, but it's still nothing like D&B or Techno.

As cool as that is for a real treat watch the opening scene from 'Lara Croft, Tomb Raider'. It was one of the first flics I watched after making the first TableTuba.

Between 21" and 24" not huge with four cabs. Remember you can use scraps you have lying about for the braces.

Theaters don't go after sub 20 Hz content, not even IMAX. The size and cost of the gear is excessive. For the most part they're content with 30 Hz.

With respect to rotary subs, it was a thing many decades ago, when we didn't have the low frequency drivers we've had since the turn of the century. Now they're a curiosity, no more relevant than dial up internet access.

The Lavoce WAN 123.00 is very close to the 3012HO and may be less expensive in some locales. The WAF 122.50 is considerably less expensive and also works quite well, it's just heavier.

No measurable difference either, so long as the bottom of the cab is less than 1/4 wavelength from the ground. At 100Hz that's 85 Cm.
